The accident took place when he was doing a routine check-up of equipment at the plant, and walked into an area that, though cordoned off by scaffolding, did not have any warning or danger signs affixed in the area. These were necessary, as there was a hole in the walkway due to the removal of a floor plate to facilitate cleaning. The warning signs’ absence encouraged the employee to enter the area, and he slipped through the gap to fall into the basement about ten feet below, sustaining serious injuries as a consequence.

One of the HSE’s major complaints has been that cleaning had been completed about ten days before the accident, but the floor plate had not been replaced, as it should have been as per the proper safety standards.
